Unlike spot trading, limit orders rely on a counterpart known as a ‘taker’ to fulfill the order. If by the time the price reaches your limit order price, the order is not profitable for takers, then they can choose not to fill the limit order.

Alternatively, setting a price during periods of volatility might mean that even if the price was reached, it did not stay at that level long enough for your order to go through.
